Align · Dental Equipment · $20,000 – $40,000
A unit with under 800 handpiece hours and full calibration runs $35k–$40k; anything over 1,200 hours or with a 18+ month service gap drops to $20k–$28k. Calibration drift repair from an authorized tech runs $900–$1,400 if not included in the sale.
Plug it in and run the self-calibration file - takes 3 minutes and tells you immediately if the optics are drifting. Inspect the handpiece tip for micro-scratches under a loupe; even hairline marks degrade scan clarity. Pull the service logs from the seller's Align portal before closing; if they can't access it, walk.
California DSOs are upgrading from Element older generations to 5D because insurance reimbursement for NIRI-detected lesions is climbing. Texas and Florida chains are buying used units aggressively to compete with DSO pricing in high-volume markets.

Not marketing. NIRI catches 23–27% more interproximal lesions than optical scanning alone per published Align studies. Western Dental measured a 6% uptick in preventive case acceptance after switching to 5D, which is real dollars.
Staff picks it up in 2–3 days if they've used older iTero models. Align won't train on used machines directly, but any decent DSO tech can walk your team through it in 4 hours. I usually bundle a 90-minute remote walkthrough in my deals.
You can upgrade the operating software, but it has to run through an authorized Align service center - roughly $600–$800 for the update plus calibration validation. Sage Dental does this in bulk quarterly, so if you buy from a chain, ask if they just updated.

I save buyers 6–8 weeks of vendor quotes and demo waiting by vetting units pre-sale and confirming calibration status. That's real money when a chair sits empty - roughly $2,500 per week in lost production for a typical group practice.
